John Jack Velazquez, age 94, of Clermont, formerly of New Port Richey, passed away November 1, 2018. He was born in Brooklyn, NY to Juan and Sara Velazquez. Jack served proudly in the US Navy during WWII.  He is survived by his loving family.

Military honors will be given 11am November 26 at FL National Cemetery. Arrangements by Michels & Lundquist.
